Visiting Nurse and Community Health clinicians provide a wide range of comprehensive professional health care, so patients are able to stay in the comfort of their own homes. Working closely with the primary care physician and multidisciplinary team, a VNCH primary clinician serves as the patient’s case manager. The team-model approach gives our patients and families specialized, individual care and clinical consistency.

Home Care:

Visiting Nurse and Community Health provides highly skilled services that allow our patients to stay in the comfort of their own homes. Working closely with the primary care physician and our multi-disciplinary team, we provide the proper education, expertise, knowledge, and the proper medical/rehab regimen in order to ensure their health, well being and safety.

Home Care Plus:

Visiting Nurse and Community Health’s private-pay subsidiary, Home Care Plus, provides the continuity of care and professional services for patients who need assistance beyond what health insurance programs generally cover. Health care providers are available 24 hours a day, seven days a week.


Hospice care focuses on the patient and family.  The goal of the professional Hospice team is to empower the patient to make their own choices with its support and assistance.  The team serves as advocates, helping to access the information and resources you need during this challenging time.


HomMed is VNCH’s TeleHealth solution. It is a state-of-the-art health monitoring system offered exclusively by Visiting Nurse and Community Health. This program allows us to identify any deviation in vital signs that may be occurring. Identifying these trends allows us to react to a possible health threat before it becomes a problem.
